Community Bridges

Community Bridges is an outpatient program for brain injury rehabilitation with residential and day treatment and outpatient multidisciplinary therapies.We serve adolescents and adults with mild to severe brain injury, including those who have experienced concussion, closed or open head injury, anoxia,stroke,seizure disorder or other injuries.About 15 percent of those diagnosed with a mild injury struggle with life-long complications. Community Bridges makes living with these complications easier with a specialized program of therapies.

Finally, we assist in locating housing, community support and referrals to appropriate agencies, such as the state’s Vocational Rehabilitation Service.
