Appointments

Appointments at Rocky Mountain Perinatal Associates are usually by referral only, from the primary caregiver (obstetrician, family practitioner, or midwife) providing your prenatal care. If there are any questions about scheduling a visit with us, please call the appointment line, and your questions will be answered.

Appointments are scheduled between 8:00 a.m and 4:30 p.m., Monday through Friday. Appointments for some procedures are limited to certain times and days, so that the laboratory can perform the testing in a timely fashion. We willl make every effort to accommodate your schedule.

Please arrive for your your appointment 15-20 minutes early, so that appropriate paperwork and forms can be completed.

Please bring:

  • A printed requisition form (available in PDF format below) or other form signed by your physician requesting services from Rocky Mountain Perinatal Associates
  • picture ID
  • Insurance card
  • if required, referral authorization or other forms required by your insurance company

The only preparation for your visit (if you will be having an ultrasound) is to drink about 16 ounces of water or other liquid in the 30-60 minutes before your appointment. No changes in food intake are necessary before your appointment.

A loose top that allows easy exposure of your lower abdomen is helpful, if you will be having an ultrasound.

We respectfully request that children under the age of 10 be accompanied by another adult.

If possible, please limit guests to 2 per patient.

If you arrive more than 15 minutes late, it may be necessary to delay or reschedule your appointment; if you anticipate being late, or need to cancel or reschedule, please contact us as soon as possible, so that we can more easily accommodate you. If, because of unforseen delays, our office is running behind, we will make every attempt to contact you before your appointment so that you can reschedule or be accommodated in whatever way works best for you.
