The Student Affiliation Program is designed for students who are currently enrolled in a school-sponsored, credit-bearing course or academic program and are seeking a clinical rotation at Community Medical Center (CMC). Participation requires a formal affiliation agreement between the educational institution and CMC. If your school does not currently have an agreement in place or if you have questions regarding the process, please contact us for assistance.

An active affiliation agreement is required before any student placement or clinical rotation can be approved. Establishing or renewing an agreement may take time, so schools are encouraged to contact us as early as possible to confirm that an agreement is current and in place well in advance of the planned educational experience. Individual departments are not authorized to complete this process; all agreements must be reviewed and approved by the Student Affiliations Department.

For RN-to-BSN and graduate-level nursing students, a current and unencumbered Montana RN license is required. Nurses who hold an active license in a compact state may participate in a clinical rotation for up to 30 days. If the RN does not hold a compact license, a Montana RN license must be obtained. Clinical rotations may not begin until the appropriate nursing license is active.

Paperwork, Documentation and Requirement Information

After confirming an Affiliate Agreement is in place, please proceed to step 1.

Step 1. Student Clinical Placement Request – This form needs to be filled out in its entirety and returned to the director of education at CMC by the nursing program instructor/coordinator.

Once a placement has been approved, our department will notify you to complete the paperwork outlined in steps 2-4 below.

Step 2. Student Documents – Once placement is secured, complete these documents.

Step 3. Informational Manuals - This step is for the student to keep and does not need to be returned.

Please Note: Compliance Training courses are required for all students/residents who complete 160 or more clinical hours at CMC. Please contact us and request a LifeTalent account be set up for you to complete these modules.

School Instructions

CMC requires all students/residents to be fully vaccinated against COVID-19 or have obtained an approved medical or religious accommodation approved by your school.

Step 4. All School Forms (excluding the placement form that should already be approved)

• Fill out the school portion of the Background Release Form after the student has filled out their portion of the form unless the student is providing a copy of the background check with the release form. The background must include a national sex offender search and an exclusions list check as well as the national criminal background check Fill out the Background Release Form in case we need to obtain a copy of the background check from the school for an audit or submit a copy of the background check with the Background Release Form. In some cases, the school may not provide a background check for students. If this is the case, the student will be responsible for providing a criminal background check along with exclusion list checks from the national sexual offender registry search (https://www.nsopw.gov/ or https://doj.egovmt.com/choprs/) Please check with your school contact to see if this is your responsibility to avoid delays in starting your student experience with CMC.
  
• Any documents that need to be filled out (not including the documents for the student to keep) need to be turned in and approved two weeks before the start of rotation. Once student and school forms have been accepted, then our department will reach out to discuss orientation needs i.e. badges, post clinical conference room reservations, electronic medical record training etc. The student must present a state or government issued ID in order to obtain their CMC ID badge. Upon completion of the clinical year, ID badges must be turned back in to the onboarding coordinator at CMC.
